Pharmacy

Buy medicine, fill prescriptions, or run your own pharmacy with licensed pharmacists, upgrades and deliveries.

Jobs & Businesses8 min read

Pharmacies sell over-the-counter (OTC) medicine, snacks, drinks and hygiene items, and fill the prescriptions doctors write. The city runs Dollar Pills in Davis with an NPC clerk, and players can open their own pharmacy business.

This guide covers both sides: shopping and picking up prescriptions, and working at or owning a pharmacy.

None in the city right now.

Before you begin

  • To shop: nothing but money. Pay with cash or card.
  • To fill a prescription: a prescription from a hospital doctor and your ID.
  • To work there: get hired (see Employees & Hiring). Ranks are Cashier, Pharmacy Tech, Pharmacist, Manager and Owner.
  • To sign off prescriptions: a pharmacist licence, granted by your owner / manager or an on-duty hospital doctor.
  • To own one: a Pharmacy business, handed out by staff (ask in a Discord ticket). See Owning a Business.
1

Shopping

  1. Walk to the till and press E.
  2. The shop opens with aisles: Pain & fever, Cold & allergy, Stomach & sleep, Vitamins & energy, First aid, Gum & nicotine, Snacks, Drinks, Hygiene & beauty and Supplies.
  3. Pick an item, choose how many and pay with cash or card.

A yellow bar at the top means an NPC clerk is serving you (Dollar Pills, or a player pharmacy that is closed). NPC prices are the city price plus a markup. At an open player pharmacy you buy from their stock at the owner's prices, and each item shows "N left".

ItemWhat it does
Payne-Away and other pain pillsEases pain for a while
WakeUp! Caffeine TabletsFree sprinting for a short time
DozeOffA bit of stress relief, drowsy screen
Vita-Max MultivitaminsA little health
Morning MercySobers you up
Hackless Cough Syrup / Tum-CalmPauses flu / stomach symptoms for a while (doesn't cure them)
Frostbite Mint Gum and other gumA pack of 10 pieces
QuitPal Nicotine PatchesA box of 7 patches

Use one thing at a time - using two items within a few seconds says "One thing at a time".

Rewards card and gift cards

  1. At any till press Get a free rewards card. Points are saved on your character, so you can get a new card if you lose it.
  2. Carry the card when you buy to earn points. Each pharmacy sets its own earn rate.
  3. At checkout tick Use points: 20 points = $1 off, and points can pay at most half of a purchase.

Pharmacies with the photo kiosk upgrade also sell gift cards at the till. A gift card only works at the pharmacy that sold it, and whatever is left stays on the card.

3

Picking up a prescription

  1. A hospital doctor writes your prescription. You get a message telling you to pick it up at any pharmacy with your ID.
  2. At the till open the Prescriptions tab (or the Rx window at a player pharmacy). Each prescription shows the drug, dose, doctor, refills left and price. Controlled drugs have a red Controlled tag.
  3. Choose how to fill it:
  • Drop off (when staff are in): it goes into the pharmacist's queue. When it's ready, a "Ready to collect" window pops up and you pay.
  • Clerk fill (no pharmacist, or at Dollar Pills): stand at the counter for about 30 seconds. It costs more. Moving cancels it with no charge.
  • Deliver (if offered): staff bring it to you for a delivery fee. Type /rxdelivery to see the status or /rxdelivery cancel to cancel.
  1. You get an Prescription Bottle labelled with your name, the drug, doses left, refills, doctor and pharmacy.
  2. Use the bottle → Take a dose. You have to wait between doses.

Health insurance from a hospital kiosk pays part of your prescriptions. A bottle with someone else's name on it only works at half strength, and carrying it is a crime.

4

The clinic and photo kiosk

  • Clinic (pharmacies with the Clinic room upgrade): a licensed pharmacist offers flu shots, vaccines (Tetanus, Hepatitis B, MMR, Shingles) and blood-pressure checks. Press E at the clinic to see your vaccine record or show it to someone.
  • Photo kiosk: paste a picture link (https .png / .jpg), add a caption and print a Photo Print.
5

Filling a prescription (staff)

  1. Clock in and press E at the Rx window ("Rx counter (staff)"). Open the Rx queue tab.
  2. Pick a patient and work through three steps:
  3. Check ID - the patient's ID card shows.
  4. Count - click the pills on the tray. They go into the bottle in groups of 5. Clicking the empty tray is a miss and costs time. Count them all before the timer runs out.
  5. Sign off - licensed pharmacists only. The label prints and the patient is asked to pay.
  6. Or press Refuse to hand it back without using it up.

Techs without a licence can do steps 1 and 2. Controlled drugs also need the controlled-drugs permission and an Rx safe in the pharmacy.

Handwritten slips show a red Verify slip panel when you check ID. It tells you if the doctor is on record and if there's a matching prescription. If it looks wrong, Refuse + call police. A pharmacy that fills a forged slip can be audited later and fined from its balance.

6

Deliveries (staff)

  1. New delivery orders ping on-duty staff.
  2. Rx window → Rx queue → Deliveries → Take it. You get a sealed bottle and a GPS route that follows the patient.
  3. Stand next to them and press E to hand it over.

Pharmacists can also set up a delivery for a patient who phones in. Controlled drugs are never delivered.

7

The pharmacist licence

Only a licensed Pharmacist can sign off a prescription. The owner or a manager grants it on the office computer → Pharmacy → Pharmacist licences → Grant, or an on-duty hospital doctor can grant or revoke it.

8

Setting up your pharmacy (owner)

  1. Office computer → Locations: place your Front till (customers buy here) and your Rx window (prescriptions are dropped off here).
  2. Stock the stock room on the Orders tab and set your prices in Settings → Prices.
  3. Buy upgrades on the Pharmacy page and place their spots on the Locations page.
  4. Open the pharmacy.

Place your Front till inside Dollar Pills and you take that store over: its NPC clerk and blip go away and it becomes yours.

On the Pharmacy page you also find: your KPIs, the Rx queue, pharmacist licences, prescription prices, the controlled log, items running low, recent sales, robberies and audits, Services & rewards prices and Weekly deals (up to 5 discounts for 7 days, shown as an orange banner at your till - place a Weekly deals sign at the door).

  • Prescription prices: set between half and three times the city price, or Reset.
  • NPC prices: 25% over the city price at the till, 35% over for a clerk-filled prescription.

While your pharmacy is closed, an NPC clerk sells at your till and you get nothing - the Pharmacy page counts those sales as "Lost to the NPC".

An unattended till can be held up, and an Rx safe can be cracked. Losses come out of your balance or your safe. Keep someone near the till while you're open - you get an alarm when something happens.

Common problems

  • "One thing at a time" - you used two items within a few seconds. Wait a moment.
  • A supply item is refused - syringes, vials, pill bottles, scales, baggies and baking soda are limited per character per day across every pharmacy. Having a real prescription allows a bit more.
  • Your prescription isn't listed - it is waiting for its refill time, or all its fills are used up.
  • Can't sign off a prescription - you need a pharmacist licence. Ask your owner, a manager or a hospital doctor.
  • Can't fill a controlled drug - the pharmacy needs the Rx safe upgrade and you need the controlled-drugs permission.
  • A delivery was cancelled - nobody took it in time. Drop it off or use the clerk fill instead.

Every controlled fill and every capped supply sale is written in the controlled log, which police can read. Filling controlled prescriptions from several different doctors in a short time flags a patient for doctor shopping.
